About Us

Senior Care Partners, a private non-profit PACE (Program of All-Inclusive Care for the Elderly) organization, was formed in 2008 in compliance with state and federal regulations, as well as in conjunction with the National PACE Association, with the mission of providing comprehensive care for the frail elderly population of our region in order to allow them to remain safely in their home for as long as possible.
